GROUP HOMES:
- Every applicant for admission to JARC will be screened relative to the following admission criteria:
- Be 18 years of age or older;
- Have a documented primary disability of mild or moderate mental retardation but may have a secondary disability such as epilepsy, cerebral palsy, etc. This program does not meet the needs of persons with severe emotional impairments;
- Possess basic self-help skills to such a degree that he/she can care for most of his/her own self-care needs, assist in the maintenance of his/her own living quarters, care for personal clothing and share in household responsibilities;
- Demonstrate the adaptive behavior and social skills necessary to succeed in our environment;
- Upon admission, must not require physical restraint for safety purposes;
- Receive a determination that living in a JARC group home will be of benefit and in accordance with assessed needs;
- Participate in meaningful day programming that has been agreed upon by JARC staff, the family and the resident
- Provide a recent medical and psychological evaluation (within six (6) months prior to application) and other pertinent data as requested;
- Consent to receiving a JARC assessment battery which may include evaluation of adaptive behavior and medical psychiatric and/or psychological reports.

SUPPORTED LIVING:
|
- Every applicant for admission to JARC will be screened relative to the following admission criteria:
- Be 21 years of age or older;
- Desire an independent lifestyle,
- Be capable of living and sleeping alone without staff supervision present in the apartment,
- Can identify emergencies, distinguishing between emergency and non-emergency situations and is capable of calling for help,
- Upon admission, poses no threat to self or others, displays no loud or disruptive behavior,
- Performs all ADL (feeding, bathing, dressing). If prevented by a physical disability, provides a personal aide to accommodate the need,
- Demonstrates reasonable precautions regarding personal safety (locking doors and windows, not letting in and talking to strangers or hitchhiking, etc.)
- Follows apartment safety rules regarding electrical outlets and appliances, stove, oven matches, etc.
- Follows medical orders, including taking medication without staff assistance.
